Information from state Health Department records for inspections Aug. 11, through Aug. 26. Restaurants are listed in alphabetical order. All reports are from regular food service / food store inspections unless otherwise noted.
• Critical violations are defined as items that relate directly to factors that lead to food-borne illness, and must be corrected immediately.
• Noncritical violations are defined as items that relate to maintenance of food operations and cleanliness.
Aug. 11
La Huerta
129 Fowler, Ste. B, Gentry
Critical violations include: Repair cold water handle at hand washing sink. Maintain cold food at 41 degrees or below.
Noncritical violations include: Do not reuse single use containers. Defrost and clean freezer interior. Repair freezer lid, no insulation and exterior is rusting.
Chick-fil-A
2601 W. Pleasant Grove, Rogers
Noncritical violations include: Store food in freezer at least six inches off the floor, walk in too. Repair / replace paper towel dispenser at chicken breading station. Review cooling policies with employees.
Eastside Elementary
Rogers
Critical violations include: Cooler seems to be holding food at 43 degrees, needs to be 41 degrees or below. Cooler needs to be adjusted to hold foods at 41 degrees or below. At time of inspection, cooler was entered several times and door was opened several times. Temperature should be set to compensate for possible warming during heavy use times. Maintenance is working to replace compressor, parts on order.
Noncritical violations include: Leak around faucet at three-compartment needs to be repaired.
Grammy’s Carriage Daycare
303 E. New Hope, Rogers
Critical violations include: Eggs being stored above ready to eat foods, must be stored below all ready to eat foods, corrected. Refrigerator holding foods above 41 degrees. Cold holding is 41 degrees or below. Refrigerator was adjusted to be colder.
Mad Pizza Company
2603 W. Pleasant Grove, Rogers
Critical violations include: Quaternary ammonia should be 200 ppm.
Noncritical violations include: Clean inside prep table units and walk-in units.
Monte Ne Inn
9944 Monte Ne Estates Dr., Rogers
Critical violations include: Contact pest company to come up with a pest control plan that works for this location. Cool foods from 135 degrees to 70 degrees in two hours, from 70 degrees to 41 degrees or below in four hours. Cooler temperate is not maintaining food at 41 degrees or below, adjust as necessary (going down in temp during inspection ).
Noncritical violations include: When ice is used to keep food cold, ice must be above the level of the food to be effective. Scoops and in use utensils must be stored handle up out of food. Repair hot water faucet / handle at handwashing sink, part on order. If food prep staff wears finger nail polish then gloves must be worn.
Monte Ne Store

Noncritical violations include: Clean interior of cappuccino machine. Need thermometer in small sandwich cooler, corrected. Need single use towels or paper towels for handwashing sink.
Starbucks
2605 W. Pleasant Grove, Rogers
Critical violations include: Wiping cloths for the nozzle must be stored in sanitizer. Sanitizer concentration should be 200 ppm, not to exceed 400 ppm for this brand only, test exceed 400 ppm. Employees must wash hands prior to mixing drinks when changing tasks. Employee picked up towels off floor and when going to make customers drink did not wash hands.
Noncritical violations include: Do not store ice scoop on top of ice machine. Store scoop in or on something that can be sanitized daily. Storing in the ice bucket is OK. Clean out refrigerator. Milk spilled on bottom, smells sour. Clean under cabinets, excessive accumulation of debris on floors.
Subway

Critical violations include: Repair sanitizer mixing unit. Sanitizer should be 200 ppm.
Walgreen’s

Noncritical violations include: Clean under milk shelves.

Siloam Springs Children’s Center

Critical violations include: Bottle of bleach water in infant room is too weak, needs to be 50-100 ppm.
Siloam Springs Aquatic Center
1800 N. Mt. Olive, Siloam Springs
Noncritical violations include: All utensils must be stored with handle up. Container of utensils next to microwave have handles stored down.
Aug. 19
Arby’s
3929 W. Walnut, Rogers
Critical violations include: Cooler at prep stations are holding foods at 42 degrees. Must maintain foods at 41 degrees or below. Cooler next to slicing station also has condensation dripping into cooler. No foods are being stored where water can drip.
Noncritical violations include: Food items removed from original container that are not easily recognizable must be labeled, corrected.
Central Child Care Center
2535 W. New Hope, Rogers
Critical violations include: Refrigerator door is holding at 42 degrees. Must maintain foods at 41 degrees or below for proper cold holding.
Noncritical violations include: Containers of sugar in store room must be labeled with contents. Once a bulk food item is removed from its original container, it must be labeled if it is not easily cleanable, corrected. Refrigerators need thermometers.
Chick-fil-A
4001 W. Walnut, Rogers
Critical violations include: Several items from cooler next to fryer were found out of proper cold holding temperatures. Items ranged from 42 to 44 degrees. Must be maintained at 41 degrees or below. Walkin cooler was holding food at 42 to 43 degrees. Must maintain at 41 degrees or below.
Noncritical violations include: Coolers near fryers for potatoes need thermometers, corrected.
Despensa Stephany’s
105-A 21 st, Rogers
Critical violations include: Eggs stored above vegetables. Needs to be stored below any ready to eat foods.
Kum & Go
1701 W. New Hope, Rogers
Critical violations include: Container of tuna salad in cooler in prep area was 53 degrees. Manager stated that he had received it off truck. Must discard tuna salad due to temperature issue, corrected. Chemicals being stored next to food item. Must be stored separately from food.
Noncritical violations include: Thawing sausages on counter. Thawing must be done either under cool running water or under refrigeration, corrected.
Lin’s Garden
2101 SE Walton, Bentonville
Critical violations include: Beef sitting under prep table. Employee drink on service line without lid or straw. Employees need to wash hands before handling food and after touching their person, before putting on gloves and when changing gloves. Handwashing sink by employee bathroom does not have hot wash nab. Utensils in west handwashing sink by dishwasher. No signs for handwashing posted at handwashing sink. Unlabeled cans in cooler by dishwasher, bent and corroding. Employee food needs to be separate from restaurant food. Food prep areas not sanitized, using dry wiping cloths to wipe down prep tables. Beef 59 degrees, cold holding at 41 degrees or below. Need to date mark ready to eat food that are more than 24 hours old. Sanitation bucket 0 ppm, chlorine should be 50-100 ppm. Personal items need to be stored separate from restaurant items. Sushi 63 degrees, cold holding 41 degrees or below. Eggs at room temperature, need to be stored in refrigerator. Need thermometer in coolers and freezers. Live roaches in kitchen. Dead flies on fly strips in kitchen, also live flies in kitchen, multiple fly strips. Wiping cloths not stored in sanitation buckets. Tongs for food service need to be stored handle up. Gloves need to be changed after touching person. Gauges on dishwasher do not work. Outside facilities need to be cleaned. Wastewater from cleaning kitchen needs to go down drain, not into parking lot. Excess material in back of restaurant needs to be removed or organized to prevent pests.
Rupuseria Salvadorena 1
105-D 21 st, Rogers
Critical violations include: Chicken sitting over shrimp, poultry products must always be stored below other meats. Cooler is holding food at 42 degrees, must hold food at 41 degrees for cold holding, adjusted cooler down. Beans used to make pupusas was sitting out at 52 degrees while pupusas are being prepared. Must either be kept at 41 degrees or below or must use time as a control. Employees must wash hands when switching tasks or prior to glove use.
Noncritical violations include: Handwashing sink had fruit and bowls in it. Must be accessible for handwashing at all times. Must only be used for handwashing. Chicken sitting out must be thawed under cool running water or in refrigerator. Foods removed from container that are not easily recognizable must be labeled. Several items were sitting directly on floor. All food items must be stored six inches or above off ground.

Sanitize all utensils after washing and rinsing.
C & D Concessions
Noncritical violations include: Relish must be refrigerated, label states that it must be refrigerated after opening, corrected. Hot dogs were 122 degrees, must be 135 degrees for hot holding, turned up temp on crock pot. Sanitizer tested 0 ppm, must be 50-100 ppm, corrected. Cheese in cooler was 57 degrees, must be 41 degrees for proper cold holding.
C & D Concessions # 8
Critical violations include: Need paper towels at handwashing sink, corrected.
Fun Time Shows
Critical violations include: Need to wash hands before touching food contact surfaces after handling money. Corn dogs 120 degrees, hot holding 135 degrees or above. Must use within four hours of being removed from fryer.
Land S Concessions
Critical violations include: Catfish 66 degrees, cold holding 41 degrees or below. Catfish must be all used within two and a half hours.
Olde Tyme Foods
Noncritical violations include: Need to label sugar, corrected.
Papa Murphy’s
Critical violations include: Sanitizer bottle tested too high. Must be 50-100 ppm for chlorine sanitizer, corrected.
The Rolling Onion
Critical violations include: Eggs need to be stored below ready to eat foods.
Vanessa’s Summer Ice Sno on the Go
Critical violations include: Sanitizer tested 200 ppm, needs to be 50-100 ppm.
TX Twister
Noncritical violations include: Label container with product out of original containers, corrected. Need scoop with handle for sugar.
Unlimited Concessions
Critical violations include: Sanitizer tested too weak, needs to be 200 ppm for quat, corrected. Need test strips on site for quat sanitizer.
Noncritical violations include: Ice scoop must be stored handle up in ice bin.

All In One

Critical violations include: Polish sausage 120 degrees, cheddarwaurst 117 degrees, corn dog 114 degrees, hot holding is 135 degrees or above.
Cooper Elementary

Noncritical violations include: Single serve items must be stored handle up.
Las Palmas
200 W. Poplar, Rogers
Critical violations include: Raw meat and eggs stored improperly in walk-in cooler, corrected. Guacamole, sour cream were out of proper cold holding, needs to be 41 degrees or below. Food previously prepared and used at later date must be date marked.
Noncritical violations include: Items like flour, sugar, salt, etc. that have been removed from original container must be labeled. Drain from soda fountain needs to be connected to floor drain so water is not standing under sink. Must divot drain line closer to floor drain. Areas of sheet rock on wall are peeling. Need to be repaired so they are smooth and easily cleanable.
Lucky C Mart
2401 W. Olive, Rogers
Critical violations include: Items in hot case found out of temperature. Proper hot holding temperature is 135 degrees. Foods must be maintained at 135 degrees or above. Milk cooler was 45 degrees, cold holding is 41 degrees or below. Evidence of possible rodents found in cabinet, check with contracted pest company.
Noncritical violations include: Ceiling tiles missing in store room. Need to be replaced, down due to electrical work being conducted. Floor in walk-in needs to be cleaned. Floor has buildup under racks.
Sonic

Critical violations include: Ranch 63 degrees, cheese 63 degrees, cold holding is 41 degrees or below.
Noncritical violations include: Ice scoop needs to be stored in a cleanable container. Floor in cooler and freezer needs to be cleaned. Scoop in bulk product needs to be stored handle up or in a cleanable container. Jewelry may not be worn by food handlers. Hair restraints need to be worn if facial hair is long.
Subway

Noncritical violations include: Need to clean build up between soda syrup shelves and mop sink.
Aug. 26
Acambaro # 13
150 Curtis, Ste. B & C, Pea Ridge
Critical violations include: Raw chicken stored above ready to eat foods. Must be stored below ready to eat foods to prevent possible contaminants. Employee drinks in prep area / food area must have lids. Previously prepared food items placed in cooler and used at later date must be date marked and used within seven days.
Noncritical violations include: Meat being thawed sitting out at room temp. Must be thawed either by placing in refrigerator or under cool running water. All bulk food items removed from original container must be labeled. Leak on sink in dishwashing area must be repaired. Walls need to be cleaned, have excessive amount of grease build up and area around sink has possible mold. AC vent in kitchen has excessive build up and must be cleaned.
Doc’s Eat Place

Critical violations include: Raw meat was stored above ready to eat foods in walk-in. Must be stored below to prevent possible contamination, corrected.

Critical violations include: Items in walkin cooler found above proper cold holding temperature. Must maintain cold foods at 41 degrees or below. Door does not make good seal when closed.
Noncritical violations include: Leak on sink next to dishwasher needs to be repaired. A few areas on the walls need to be repainted or sealed. No longer smooth and easily cleanable. Light bulb above oven needs shield or needs to be shatterproof bulb.
